Community-Based ANP (Home Visits)

Job Description

This role is perfect for ANPs who enjoy community work. You will provide home visits and care home assessments, supporting elderly and vulnerable patients.

Key Duties

• Undertake home visits for housebound patients.
• Manage long-term conditions and palliative care cases.
• Provide urgent on-the-day assessments in the community.
• Liaise with district nurses, GPs, and families.
  Maintain accurate and timely patient documentation.

Requirements

• NMC registration with ANP qualification.
• Independent prescribing (V300).
• Experience in community or elderly care desirable.
  Full UK driving license and access to car.
